Substance Use History
We will ask about your history with substance use to create an accurate overview of your health and experiences. Please note that certain medications may require a detoxification period before participation, ensuring all participants are in their most natural state.

Mental Health and Well-being

Your mental health is very important to us, and during the interview, we will also discuss the following:

Previous Suicide Attempts or Recurring Suicidal Thoughts
We will ask if you have experienced any past suicide attempts or ongoing suicidal thoughts. Please be aware that recent suicide attempts or persistent suicidal thoughts are considered contraindications for our ceremonies, as the safety and well-being of all participants is our top priority. We encourage you to be open and honest about your experiences so we can provide the necessary care and support.

It is important for us to understand your intentions for the ceremony. During the intake interview, we will ask about:

Purpose for Participation
We will ask what your intention for the ceremony. This helps you set clear and meaningful intentions for your ceremony.

Follow-up Call
Once your participation is confirmed, we will schedule a follow-up call two weeks before the ceremony. This call will allow us to dive deeper into your personal intentions, including your family dynamics, upbringing, and any past traumatic experiences that may arise.
